Pre-Registration Information
- The child must be five years old on or before September 1, 2026
- Please bring proof of age
- Official birth certificate or other documentation with child’s birth date
- Custody papers (if applicable)
- Alabama immunization card (blue card)
- May be obtained at Alabama Health Dept. at 3400 McClellan Boulevard, Anniston, AL 36201
- Social Security number (Voluntary)
- *Disclosure of your child’s social security number (SSN) is voluntary. If you elect not to provide a SSN, a temporary identification number will be generated and utilized instead. Your child’s SSN is being requested for use in conjunction with enrollment in school as provided in Ala. Admin. Code §290-3-1.02(2)(b)(2). It will be used as a means of identification in the statewide student management system.
- Proof of residency (recent power bill and water bill with address)
- Mortgage or lease agreement
- NOTE: The address on recent power bill, referenced above, and address on the mortgage or lease agreement must match.
